Thank you for submitting your application to Hawaii IPA! We are pleased to welcome you onboard. In order to finalize your application for HMO membership, please download, sign, and email the HMO attestation sent to you via email or available on the application page to administration@hawaiiipa.com. The Anti-Trust Policy should be signed and retained for your own records.
Once received, and as long as credentialing with HMSA is already complete, you will receive a letter from HMSA in 6-8 weeks notifying you of your official effective date with the HMO. If you are not yet credentialed, please allow 6-8 weeks from the time of credentialing for such a letter.
